Alzheimer's Society are working with NHS South West London, Croydon Council and other local partners to create a joint health and social care Dementia Strategy for Croydon - making Croydon a place where people affected by dementia can live with help and hope.
But in order to meet local needs, and to shape the strategy around its residents, we need your help. We want to hear from as many people affected by dementia as possible. We would be grateful if you could forward the links below to people living with dementia and their carers. Where possible, you may want to assist some people affected by dementia to complete the survey.
