MERGER OF CROYDON PRACTICES: H83018 SELSDON PARK MEDICAL PRACTICE WITH THE H83040 MITCHLEY AVENUE SURGERY

Attached is a letter recently sent to patients at Selsdon Park Medical Practice and Mitchley Avenue Surgery in Croydon.

In summary, with effect from 1 April 2022, H83040 Mitchley Avenue Surgery will become a branch site to H83018 Selsdon Park Medical Practice and the practice contract code, H83040 will be terminated.

Both sites will remain open for GP primary care services and the two patient lists will become one on 25 May 2022 when IT can complete this stage.
